Atlas Research is currently engaged in a contract with the VA Capitol Health Care Network to provide homeless and homeless prevention services in the communities surrounding four community-based outpatient clinics. The project employs four Master’s level clinical case managers to identify homeless or at-risk for homelessness Veterans; help determine service needs; enhance Veteran enrollment; and facilitate clinical assessment and consultation.
The Rural Homeless and Homeless Prevention Services Project spans four highly rural service areas where community-based organizations are best suited to understand and develop the lasting relationships necessary to break the cycle of homelessness. Case managers partner with VA to ensure Veterans receive timely medical care, psychiatric or substance abuse treatment services, residential treatment services, and/or social-vocational assistance. They interface directly with Department of Housing and Urban Development-VA Supportive Housing (HUD-VASH), VA Grant and Per Diem, and Domiciliary Care for Homeless Veterans programs, and are actively involved with their local Continuums of Care. Finally, case managers coordinate a broad spectrum of services outside of those traditionally provided by VA, to include social security, food stamps, transportation assistance, free cell phones, legal assistance, pre- and post-incarceration services and rent assistance.
The on-going project has demonstrated success in identifying homeless or at-risk for homelessness Veterans, serving 87 Veterans in four months. |
